With six sites across the city, you can register at any location and access care wherever suits you best. We’re always delighted to welcome new patients to the Pathfields community.

New Patient Registration

Register in minutes and choose to be seen at any of our sites, wherever suits you best.

Register with a GP Practice

If you have the NHS App, you can use it to change GP surgery.

Register in 3 simple steps

  1. Start your online form - Use our secure online registration form. It takes around 10–15 minutes per person. You can also register in person at any of our sites or fill in 
  2. Tell us about you - Have details of your previous GP and any regular medicines to hand. If you’re registering a child, a parent or guardian should complete the form.
  3. We’ll get you set up - We usually complete registrations within 3 working days. When we receive your registration, you will be sent a welcome message with a short questionnaire to fill in, this ensures we have all your important details from the start. We’ll let you know when it’s done.

Frequently asked questions

  • Who can register? – Anyone living in Plymouth or nearby can register with us. No ID, proof of address or immigration status is required.
  • Can I choose where I’m seen? – Yes. You can be seen at any Pathfields site, regardless of where you live.
  • Out-of-area registrations – If you live outside our home‑visit area, you can still register but home visits may not be available
  • How long does it take? – We usually process registrations within 3 working days.

Change of Address or Contact Number

If you are moving house, please let us know as soon as possible. If you are moving out of the surgery's catchment area, you may be asked to register with a surgery that’s closer to your new home address.

You can  to let us know about a change in your contact details or address by filling in this form Change your contact details form. There is no requirement to come into the Practice to complete this.
