Thank you for using our online consultation service, SystmConnect. This platform helps you access care more efficiently and often more quickly than by calling us and helps improve our telephone access for those without digital means of contact. 

We accept medical consultation requests—categorised into specific clinical areas from 8am, Monday to Friday (excluding bank holidays). Once we reach our safe working limit for the day, we switch to an urgent-only access protocol.

In addition to medical requests, you can also use this service to request fit notes, prescription queries, GP letters, and subject access requests.

Our triage system uses tailored questionnaires that have been developed specifically by our practice for our patients. We are committed to reviewing your request on the same day it is submitted and will respond with further information on the next steps.

Tips:

You can log in using your NHS login or SystmOnline credentials, which will pre-fill parts of the questionnaire to save you time though this is optional.

Safe Working Limits

To keep our service safe and manageable, we may need to temporarily pause access for some adult patients once we reach daily capacity. The time this happens can vary based on demand.

However, if you fall into one of the following categories, you can still submit a request through System Connect at any time until 5pm:

  • Children (under 18 years)
  • Pregnant
  • Receiving palliative care
  • Currently undergoing cancer treatment or have a cancer diagnosis
  • Registered as housebound
  • Learning disability

Each group will have a specific questionnaire to complete. Please only use the relevant form for your circumstances — incorrect use may result in your request being declined.

Pharmacy Support for Minor Conditions

You can also book a Pharmacy First appointment directly for help with the following conditions:

  1. Ear pain (ages 1–17)
  2. Impetigo (ages 1+)
  3. Infected insect bites (ages 1+)
  4. Shingles (ages 18+)
  5. Sinusitis (ages 12+)
  6. Sore throat (ages 5+)
  7. Urinary tract infections (women aged 16–64)

If you're off work sick for 7 days or less, your employer should not ask for medical evidence that you've been ill - you do not need to complete this form. Instead they can ask you to confirm that you've been ill by filling in a form when you return to work. This is called self-certification.

Have you thought about Pharmacy First? No GP appointment required and treatments such as antibiotics are available from the Pharmacist. 

Speak to your pharmacist if you have:

- Sinusitis (adults and children aged 12 years and over) 

- Sore throat (adults and children 5 years and over) 

- Earache (children aged 1 year to 17 years) 

- Infected insect bite (adults and children aged 1 year and over) 

- Impetigo (adults and children aged 1 year and over)

- Shingles (adults aged 18 years and over) 

- Urinary Tract Infection (women, aged 16 to 64)

What are the opening times for the Triage team?

Our Triage system will open at 0800 Monday to Friday

Can I put a GP letter request in at any time? 

Yes – the online consultation for letter and fit note are open all day, everyday 

How do I make a request for someone else? 

After completing any questionnaire, website users are prompted to record the details of who is submitting the SystmConnect request. Requests can be submitted by patients themselves, a healthcare professional or someone else on the patient’s behalf e.g. a relative 

Can I submit requests all day for medical issues?  

To protect both patients and staff, we’ll be working within safe working limits. This means there will be a set number of online forms we can accept each day. This helps ensure that everyone who contacts us receives a timely and safe response—and that our team can continue to provide high-quality care without becoming overwhelmed. If the daily limit is reached, the online form will temporarily close for the rest of the day. In that case, we ask you to try again the next working day when the form reopens at 8am. If your need is urgent and you’re unable to wait, you can still call the surgery for advice. 

What if I haven’t got a SmartPhone or internet access? 

Do not worry, our patient advisors can submit a request on your behalf.  

What happens if the appointment link doesn’t work or if I don’t have a SmartPhone? 

Do not worry, please just give us a call and one of our patient advisors will assist you with this.
