Thank you for using the online consultation option - SystmConnect, as this allows you to access help in a timely manner and will take less time than it would take for you to phone us.

We accept medical triage requests that are split down into Children and Adults from 8am until we have reached our safe working limit for the day, Monday to Friday (excluding bank holidays), we then revert to an urgent protocol. 

We also accept request for fit notes, prescription queries, GP letter and subject access requests. 

Triage requests are made using questionnaires that we have originally created as a Practice that are bespoke to you, our patients  and we are committed to responding to you on the same day that you submit a request with further information on how we will be dealing with your request.

If a clinician thinks the next best step is an appointment in the surgery, we may send you a booking link that will enable you to book your child's appointment yourself. If you received a link, we encourage you to book your appointment as soon as possible as the links do expire. Do not worry, if you are unable to use booking links we will book the appointment instead
Adults Online Consultation

This pathway if for Adults over 18 years old. 

We have now reached our safe working limits for the day.

This platform is open at 8am but will temporary pause on accepting medical requests when we reach our safe working limits. However, if you are in one of the following groups and you medical need is for today you may still call our phone lines to speak to one of our patient advisors:

If you are pregnant
If you are housebound
If you have learning disabilities
If you are receiving palliative care

Have you thought about Pharmacy First? No GP appointment required and treatments such as antibiotics are available from the Pharmacist. 

Speak to your pharmacist if you have:

  • Sinusitis

GP letter/Subject Access and Admin Request

This pathway is for requests for a GP letter or subject access request.

If you're off work sick for 7 days or less, your employer should not ask for medical evidence that you've been ill - you do not need to complete this form. Instead they can ask you to confirm that you've been ill by filling in a form when you return to work. This is called self-certification.

Have you thought about Pharmacy First? No GP appointment required and treatments such as antibiotics are available from the Pharmacist. 

Speak to your pharmacist if you have:

- Sinusitis (adults and children aged 12 years and over) 

- Sore throat (adults and children 5 years and over) 

- Earache (children aged 1 year to 17 years) 

- Infected insect bite (adults and children aged 1 year and over) 

- Impetigo (adults and children aged 1 year and over)

- Shingles (adults aged 18 years and over) 

- Urinary Tract Infection (women, aged 16 to 64)

Did you know you can self refer to Physiotherapy without seeing a GP first? 

Physios are great at assessing, diagnosis and treating most musculoskeletal complaints.

What are the opening times for the Triage team?

Our Triage system will open at 0800 Monday to Friday

Can I put a GP letter request in at any time? 

Yes – the online consultation for letter and fit note are open all day, everyday 

How do I make a request for someone else? 

After completing any questionnaire, website users are prompted to record the details of who is submitting the SystmConnect request. Requests can be submitted by patients themselves, a healthcare professional or someone else on the patient’s behalf e.g. a relative 

Can I submit requests all day for medical issues?  

To protect both patients and staff, we’ll be working within safe working limits. This means there will be a set number of online forms we can accept each day. This helps ensure that everyone who contacts us receives a timely and safe response—and that our team can continue to provide high-quality care without becoming overwhelmed. If the daily limit is reached, the online form will temporarily close for the rest of the day. In that case, we ask you to try again the next working day when the form reopens at 8am. If your need is urgent and you’re unable to wait, you can still call the surgery for advice. 

What if I haven’t got a SmartPhone or internet access? 

Do not worry, our patient advisors can submit a request on your behalf.  

What happens if the appointment link doesn’t work or if I don’t have a SmartPhone? 

Do not worry, please just give us a call and one of our patient advisors will assist you with this.
