Adult Social Care Support in Plymouth

Plymouth City Council's Adult Social Care service helps adults aged 18 and over who may need support to live safely and independently. Support is based on an assessment of an individual's needs and may include advice, equipment, adaptations, care at home, occupational therapy, and signposting to community services. Both patients and professionals can make referrals online through simple, guided forms.

When to Consider a Referral

Adult Social Care may be appropriate for patients who:

  • Are struggling with mobility or completing everyday activities.
  • Need support to remain independent at home.
  • May benefit from a Care Act Assessment or review.
  • Require an Occupational Therapy assessment for equipment or home adaptations.
  • Would benefit from Physiotherapy or rehabilitation support where appropriate.
  • Have increasing care or support needs due to illness, disability or frailty.
  • Require a welfare check or need support accessing emergency assistance, including food support where appropriate.
  • Have carers who may require additional support or a carers' assessment.
