If you would like to give us feedback or make a complaint about the surgery or a service you received, you can give us feedback by completing the form below.
With your consent, we use Heidi (AVT) to create a summary of your consultation. Your clinician checks it before it’s added to your record, and the recording is then deleted. Questions? Please ask us!
